Employees are able to view and print current and prior pay statements, annual W-2 forms, and change an address by using the Employee Self-Service Portal. Employee Exit - Administrators/Hiring Managers must complete this electronic form
                               to notify the Central Office of employees exiting the District.

Intent to Hire - PROFESSIONAL STAFF - Administrators must complete this electronic form to notify the
                               Central Office of all new professional staff recommended for appointment.

                               complete, print, and sign this form and send it to the Central Office with all required 
                               documentation for new professional staff hires.

Intent to Hire - NON-PROFESSIONAL STAFF - Administrators/Hiring Managers must complete this electronic form
                               to notify the Central Office of all new non-professional staff recommended for appointment.

Internal Transfer - Administrators/Hiring Managers must complete this electronic form
                               to notify the Central Office of the internal transfer of existing staff.

Leave of Absence - Administrators/Hiring Managers must complete this electronic form to notify the Central
                                 Office of employees leaving on  *OR *  returning from an approved Leave of Absence.

   Supervisor's Report of Injury - Administrators/Hiring Managers must complete, print, and sign this document, and then send it to                                                               the Central Office to be submitted to Worker's Comp when any staff member reports an injury that                                                               occurred while on the job.
